Privacy Policy

Evolution Vision (Pty) Ltd, registration number 2021/610134/07, is the responsible party for personal information collected through this website. This policy explains what we collect, why we collect it, and what you can ask us to do with it. It is written to meet the Protection of Personal Information Act 4 of 2013 (POPIA).

What we collect

We collect only what you send through the enquiry or tissue request form. That is your name, practice or hospital, email address, phone number, the line you are asking about and your message. A tissue request also carries the case details you enter, such as the procedure, surgery date and donor criteria. The form records which page it was sent from and, where relevant, the reference of the tissue listing you asked about. On the EVO ICL™ page, it also records whether you asked for the brochure.

The forms do not ask for patient information. Please do not include anything that identifies a patient.

Why we collect it

We use it to answer your enquiry, to match a tissue request against what the eye banks have, and to follow up on the line you asked about. We do not sell your information, and we will not add you to a mailing list without your agreement.

Who sees it

Your enquiry comes to Evolution Vision directly. Where a request concerns a supplier’s line or a tissue listing, we may share the details needed to fill it, such as your name, your practice and the case details, with the supplier or eye bank concerned.

This website is hosted by Framer, and enquiries are delivered to our business email on Google Workspace. Both process information on our behalf and store it on servers outside South Africa. We use service providers that protect personal information to a standard comparable to POPIA.

How long we keep it

We keep it for as long as we need it to deal with your enquiry and any supply that follows, and for the period the law requires us to keep business records. We do not keep it longer than five years after our last contact with you unless the law requires us to.

Cookies

This site does not use advertising or tracking cookies. Visitor numbers are counted without cookies and without identifying you.

Your rights

You may ask to see the personal information we hold about you, have it corrected or deleted, or object to us using it. Email enquiries@evolution-vision.com and we will respond within a reasonable time. If you are not satisfied with how we handle your information, you may complain to the Information Regulator at inforegulator.org.za.
